If payment is not received by November 16, 2020, $50 will automatically be added to the tuition.

Course Cancellation Fee

$100 December 16, 2020 to January 15, 2021. No refund after January 15, 2021; however, 50% of tuition paid may be applied toward tuition for another Northwest Anesthesia Seminars program within the next 12 months. Cancellation must be made in writing.

We recommend that you register early to assure your spot. We reserve the right to close registrations to a course at any time without notice. We may not be able to accommodate onsite registrations.

Space is limited and based upon availability. Airfare is not included. Individuals are responsible for airfare to and from Quito/Lima. No children under age 12 allowed. Children 12 and up will be charged at the adult rate. Children and non-registered guests are not allowed to attend the lectures. By registering for this course, you are agreeing to attend lectures for the entire duration of the tour. Due to the nature of the tour, partial registrations are not allowed.

Visas: US Passport holders do not require visas for Ecuador, Galapagos Islands, and Peru. Please ensure that your passport is valid for 6 months from return date and that travelers’ names exactly match your registration.

Specific Requirements:
• Valid passport for more than 6 months.
• Return ticket (for air, land or sea travel).
Nationals of the following countries require a visa to enter Ecuador for any reason and should approach the Consulate of Ecuador in their home country before traveling (list may change without notice): Algeria, Bangladesh, Costa Rica, Cuba, El Salvador, Guatemala, India, Iran, Iraq, Jordan, Lebanon, Lybia, Nicaragua, Nigeria, North Korea, Pakistan, Palestine Authority, Panama, People’s Republic of China, Sri Lanka, Syria, Tunisia, and Vietnam.

Reservations will be secured and accommodations assigned upon receipt of passenger deposit of $2,000 per person. Final payment must be received by October 10, 2020. Reservations not guaranteed until payment received.

 

No penalty prior to October 10, 2020. 100% non-refundable after October 10, 2020 for any reason.
